A medical record is a unique detailed account of the care rendered to your child by the healthcare provider. Both the hospital and your physician will have medical records related to your child's care. From time to time, you may have questions about your child's records. The release of the medical record, or any data contained within, must be approved by a parent or guardian.

Medical records can be requested for a fee by sending the following information in writing to our Health Information Management office:

• Patient name
• Patient date of birth
• Requestor's name
• Requestor's phone number
