Le Bonheur Children’s Medical Center is designed as a place for kids – from kid sized beds to grape flavored anesthesia.  We understand that surgery can be a scary experience for both parent and child, and our expert staff is specially trained to treat children.  Our staff ensures paperwork and examinations are complete so that you can concentrate on what matters most – your child.

A nurse from the Pre-op Screening office will complete pre-admission information over the phone two to seven days prior to your child’s surgery.  An appointment may be required prior to surgery if medically necessary or if lab work is needed.  If your child has a contagious disease (such as chicken pox, measles, or mumps), or has recently been with someone with a contagious disease, you must notify the surgeon’s office. 

If you did not receive the Pre-op Screen Patient Form from your surgeon’s office, you may print that form, by clicking here, and complete the information:

• Answer all questions and write NA (not applicable) by questions that do not pertain to your child
• Place your child’s name and date of surgery on every page
• Please list current phone numbers including home, cell, work and emergency numbers where a parent/guardian can be reached during the day. 
• If your phone number changes, please notify us with the new information at 901-287-6273.
• Once the form is completed, fax front and back of all pages to fax number 901-937-3335.
