About Delta Rural Health
The Delta Rural Health Initiative is a program that delivers preventative and clinical health services for individuals with or at risk of developing chronic diseases in rural Tennessee. Services are provided through the Delta Initiative include:
- Chronic Disease Care Management
- Pharmacy Assistance
- School-Based Preventative Programs
- Community Education and Outreach
In 2010, Paris Henry County Healthcare Foundation, Inc. and Le Bonheur Community Health and Well-Being was awarded the $1.53 million grant from the Health Resources and Services Administration (HRSA). This grant extends for three years, beginning August 1, 2010 and ending July 31, 2013. The primary purpose of the Delta Initiative is to provide a comprehensive network of services that focus on chronic diseases such as heart disease, stroke, diabetes and hypertension and the risk factors associated with these diseases. The programs provided through the Delta Initiative are a collaborative effort of multiple health care and social service organizations located in the 20 counties in rural West Tennessee.
Counties participating in the Delta Initiative include Benton, Carroll, Chester, Crockett, Decatur, Dyer, Fayette, Gibson, Henry, Henderson, Hardeman, Hardin, Haywood, Lake, Lauderdale, Madison, McNairy, Obion, Tipton and Weakley.
