Pharmacy Assistance Program
The Pharmacy Assistance Program is designed to match indigent individuals with pharmacy discount programs to increase compliance with prescribed care. The majority of pharmaceutical companies, in addition to Medicare, have discount programs for the uninsured, elderly, and those living in poverty. However, the paperwork is often too difficult and overwhelming for these populations to complete. The goal of the pharmacy assistance services is to guide individuals through this process based on referrals from physicians and hospitals.
The Pharmacy Assistance Program can work in partnership with the Chronic Disease Case Management Services or can be a stand-alone program.
