Medical Records

A medical record is a unique detailed account of the care given to your child. Both the hospital and your child’s doctor will have medical records related to your child's care. From time to time, you may have questions about your child's records. The release of the medical record, or any data contained within, must be approved by a parent or guardian.

At Le Bonheur Children's Hospital, we want you to be well-informed about your care so you can support your child's journey to optimal health, and that’s why we offer a patient portal that provides access to your child's healthcare information. Your access is being expanded even further.

You will be able to see even more medical records and notes in your child's patient portal based on the 21st Century Cures Act.

This Act mandates healthcare providers must increase transparency by providing you with the tools to easily access and see your child's health information. By opening more medical records and notes we believe you will benefit by: better understanding your child's care plan, being more prepared for your child's medical visits, feeling more in control of your child's care, knowing more about your child's medications and much more.

For some background, here are some items you can already access in your patient portal:

  • Current health profile (medications, allergies, immunizations, health issues)
  • Lab results
  • Patient education (assigned by your nurses or providers) and "get well" videos
  • Clinic visit summaries
  • Procedure lists
  • Diagnostics (radiology report results)
  • Clinician notes from clinic visits
  • Access to past and upcoming appointments
  • Communication tools to send/receive secure messages from your nurses or providers
  • Pre-visit data collection electronic clipboards
  • Procedure, surgical, and anesthesia notes
  • Your hospital provider and nursing notes
  • Behavioral Health Unit: labs, diagnostics, procedures, progress notes, nursing notes
  • Pathology reports
  • Clinical information received from other providers outside our healthcare system
  • Images taken (wounds, dermatology, etc.) and saved to your chart
  • Please note: There are exceptions that take into account very sensitive situations.

Extending your Medical Records to Apps

While we offer “HealtheLife” for access to your medical records, available on the Apple App Store or Google Play Store, we also support third party API integration for patient approved access.

We now offer the ability for you to securely connect third party health management applications to your health record if they are able to adhere to the industry standard Fast Healthcare Interoperability Resources (FHIR) Application Programming Interface (API). If you are interested in this opportunity please ask your health management provider, Apps Developer or Vendor to contact us at HIEInteropSupport@mlh.org.

Once we receive the request, we will contact the vendor to determine if they meet the technical requirements in order to establish a secure connection.

Medical Records

To request a copy of your medical records for yourself, healthcare provider, or third party complete a Medical Authorization Form, and provide a copy of your photo ID.

Walk-In: You may leave an authorization/request for your medical records at any MLH facility with the Registration desk associate, and it will be processed within 10 business days.

Please ensure that all fields are completed in order to ensure timely completion of your request. A copy of your valid photo ID must be included with your request. 

Written proof of authority should accompany the request in order to verify appropriate health information.

Records of Deceased Patients: To obtain a copy of a deceased patient’s record, you must complete, date and sign a Medical Authorization Form. Authorization form should be completed and signed by:

  • Next of Kin
  • Executor of Estate or Court Order Administrator
  • If there is no Executor of Estate or Court Order Administrator, a notarized Affidavit of Next of Kin form must be completed.

A death certificate and additional proof of authority may be requested, as appropriate. See FAQs and forms for related information.

Radiology Records: To obtain copies of radiology images or imaging films please contact the location where your test was performed. Le Bonheur: 901-287-6041 (inpatient) / 901-287-4560 (outpatient)


Copy Fees

Abstract copies = no charge.  There is no charge for abstract copies.  Abstract includes all pertinent documents from your stay such as ED Notes, History & Physical, Discharge Summary, Consults, Operative Reports, Radiology and diagnostic reports. 

Entire copy of Visit/Record = Paper copy = $0.07 per page & supply and postage fee  
Patient Portal= flat rate $6.50.  

Archived copy =  Paper copy -$0.07 per page & supply and postage fee
Patient Portal= flat rate $6.50

Physicians and Healthcare Requestors needing copies of patient medical records for immediate patient care, please fax request on office letter head or signed patient authorization to designated location.
