Application Process

Applicants must possess a Doctor of Pharmacy degree from an ACPE-accredited College of Pharmacy. The applicant should be licensed in the state of Tennessee within the first month of the residency program. 

The following items should be submitted via the Pharmacy Online Residency Centralized Application Service (PhORCAS) by January 1st:

  • PGY1 and PGY2 candidates should complete their respective personal statement form in lieu of a traditional letter of intent and upload to PhORCAS® as a personal statement.  Please do not submit a traditional letter of intent.
  • Current copy of curriculum vitae (CV)
  • Three references via PhORCAS®, which meet the following criteria: at least two of the three references must be from clinical practitioners with experience evaluating the candidates' direct patient care skills; if more than three references are submitted, only the first three received will be used in the initial candidate scoring assessment; see below for more details
  • Transcripts from pharmacy school

Candidates should include the following information to reference writers when requesting they complete the PhORCAS® Electronic Reference Form:

Please complete the PhORCAS® Electronic Reference Form in lieu of submitting a traditional letter of recommendation. Full letters of recommendation should NOT be included in the program-specific comments section at the end of the form. Reference writers should provide narrative comments on at least four of the evaluated areas on the PhORCAS® Electronic Reference Form as a means to provide our program with a thorough assessment of the candidates’ skill level across a broad range of areas. The program-specific comment box should be reserved for areas the reference writer believes the candidate truly excels above others and are not assessed in other areas of the PhORCAS® Electronic Reference Form.

Candidates invited for an interview will be asked to deliver a presentation.  Details will be provided when the interview is schedule.
