The Le Bonheur Endocrine Center recommends that your child’s school care plan (SCP) and 504 Plan are reviewed with school personnel each school year and with any changes.
School Care Plan (SCP) - The SCP is a diabetes medical management plan. This plan contains the provider’s orders including the insulin or medication regimen, treatment plan and individual self-care management needs. The SCP will be updated by our health care team when there are changes to the insulin regimen, level of self-care management and school circumstances.
504 Plan - The 504 Plan is a section of the U.S. Rehabilitation Act of 1973 that prohibits the discrimination against people with disabilities. This plan sets out an agreement between the school personnel and family to ensure that children with diabetes have the same access to education as all children with reasonable accommodation. The 504 Plan applies to all public and private schools receiving federal funding.
