Community Health Workers (CHWs) serve as a bridge between the hospital, clinics and the home. As part of the medical care team, we provide patients and families with support and resources.
CHWs serve as a vital bridge between patients and community resources that help them thrive. Because we are part of the communities we serve, our team brings a deep understanding of local strengths, challenges and culture, building meaningful trust with individuals and families.
Our team connects patients and families to essential health and social services, ensuring those resources are accessible, culturally relevant and responsive to to the patient’s needs. CHWs empower individuals through education, support and advocacy to help navigate complex systems and make informed decisions. Our work fosters self-sufficiency and strengthens the foundation for healthier, more resilient communities.
This program is for children who:
- Frequently access health care (sick visits, urgent care or hospitalization) due to their medical condition
- Live in Shelby County and surrounding Tennessee counties, Arkansas or Mississippi
- Are ages birth to 18
- Miss school days because of their medical diagnosis
- Struggle to participate in typical physical activity
- Have been identified by a medical professional (physician, nurse) as likely to benefit from our services
