Your Transition to MyChart
We have transitioned from MyLeBonheur Portal to MyChart powered by Epic as of Oct. 5, 2024. MyChart has replaced our existing portal, bringing enhanced features and greater convenience. You can still access the read-only version of MyLeBonheur Portal until Dec. 31, 2024.
What is MyChart
MyChart is your new, convenient and comprehensive patient portal that allows you to manage you or your child’s health information quickly and securely from one place.
Key Benefits of MyChart
- Family Access: Take care of your children and other family members all from one account.
- Preventive Care: Keep track of routine care that is due and receive reminders for flu shots and other immunizations, physical exams and preventive care procedures.
- Growth Charts: Track your children's growth and compare to others in their age group.
- Test Results: Receive timely notification of results for tests you've had done and view test results from the past.
- Get Medical Advice: Send a message to ask your doctor for medical advice and get care recommendations.
- Medication Refills: Ask for a prescription renewal and receive notification when your medications are ready to be picked up at the pharmacy.
- Request a Referral: Request a referral to see a specialist.
- Appointment Scheduling: Schedule an appointment or request the clinic contact you. View details about future appointments, including instructions and driving directions or cancel appointments you no longer need.
- Bill Payment: View billing statements and make online payments for your medical bills or visit copays through your bank account or credit card.

How to Create an Account
You can sign up for an account in several ways:
- Clinic staff can sign you up directly while you are at the front desk or in an exam room.
- A MyChart activation code might appear on your After Visit Summary, or you might receive a text or an email with an activation code after your visit.
- You can request an activation code on the MyChart website or mobile app. Click “Sign Up Now,” then click “Sign Up Online.” You can then enter personal verification information, such as the last four digits of your social security number and date of birth, and request a code, which will be sent via email or text.
After you have an activation code, you can sign up easily on the website.
- On the login screen, click “Sign Up Now.”
- Enter your activation code and personal verification information. Then click “Next.”
- Choose your login information, including a username, password and security question. Follow these tips:
- Choose a username that you can remember but that is difficult for others to guess. Your username cannot be changed.
- Create a unique password that uses numbers and uppercase and lowercase letters. It cannot be the same as your username. Make sure your password is not the same one you use on other websites.
- Choose a security question that you can easily remember the answer to verify your identity if you forget your password. The answer cannot include your password.
- You can then log in using your username and password.
Keep in mind you must be at least 14 years old to sign up for a MyChart account.

MyChart Mobile App
Download the MyChart app using the instructions below:
- Open the Apple App Store (if you have an iOS device) or the Google Play Store (if you have an Android device).
- Search for MyChart. Look for the following logo to make sure you have the right app, then install it.
- Tap Open or find the MyChart icon on your device and tap to open it.
- Follow the setup prompts, then search and select Methodist Le Bonheur Healthcare from the list of organizations.
Proxy Access
Who can create a MyChart account?
For minors ages 0-13:
A parent/guardian should create their own MyChart account in order to have proxy access provisioned to them to see their child’s information in MyChart. They can request proxy access through MyChart self-signup, HIM or with their care team. This form is linked to the pre-login page of MyChart. The parent doesn’t have to be an MLH patient to create an account.
For minors ages 14-17:
Parents’/guardians’ existing MyChart access to the minor’s portal will become limited by default when the child turns 14. Limited access allows parents to see insurance status and immunization records. Minors ages 14-17 can request their own MyChart portal in the clinic or online.
What happens when the minor turns 18?
Existing parent/guardian access will be removed when an adolescent turns 18. The young adult can then grant proxy access if desired.

Frequently Asked Questions
As of Oct. 5, the My Le Bonheur Portal will become read-only until Dec. 31, 2024. You will no longer be able to use the portal and will need to register for MyChart.
Most of your health care data will be in MyChart on Oct. 5. If you had an appointment at a Methodist Le Bonheur facility between Sept. 1 and Oct. 4, 2024 – information from those visits will transfer to MyChart by Dec. 31, 2024. In the meantime, you can look at information from those appointments (Sept. 1 – Oct.4) in your old patient portal. See the chart below with a breakdown of the information that will be available in MyChart on Oct. 5, 2024
Your family history will not transfer over to MyChart or our new electronic health record system. You will need to create a MyChart account and complete the Family History Questionnaire to help your care team provide optimized care.
Yes, once you sign up for a new portal, you will be able to send messages to your child’s physician and request test results and medical records through MyChart.
Yes, you can take care of your children and other family members all from one account.
Yes, you will be able to directly schedule appointments for some of our clinics and locations directly from MyChart. Please check with your care team to see if it will be available for their specific clinc.
