Blood Glucose Log


Patients and families use blood glucose logs to record daily and weekly blood sugar levels and help to establish patterns. Recognition of patterns leads to appropriate adjustments of meal timing, snacks and insulin therapy. Sharing the logs with your provider is one of the best forms of communication in determining needed changes in therapy between and during clinic visits.

Sick Day Log

A sick day log sheet is a record of ketone management. Patients should record blood glucose records, ketone status, amount of hydration (in ounces) and insulin doses. Keeping a sick day record helps patients determine if the sick day rules are working or if additional assistance is needed. If you have any concerns about your child’s ketone status after use of the sick day guidelines, please contact your provider.
